This link includes my original version, the Brawl- 4.1 version, and a couple of other versions as well.
From the game Super Smash Land made by Dan Fornance (supersmashland.com). Music included by flashygoodness (http://store.flashygoodness.com/album/super-smash-land-hidden-tracks). Thanks to A.J Nitro as well for ripping the sprites.
Updated in December 2017 - added an alternate based on the Brawl- 4.1 version, but without the dialogue box.
