Where do i put items? like the pokemon replacement pack or a new texture for the smash ball?

Items are to be replaced in common3.pac, in ItmCommonBrres. I don't know about PokéMon at the moment, but I can get back to you on that, but for now, I can at least tell you what the items are called.

I'm back. About PokéMon, their individual folders go into pf/item, and each folder has to be named properly. The file you put in these folders must be named as "Pkm*POKÉMON NAME*Brres.pac.
For example, a Torchic hack should be named PkmAchamoBrres.pac, and goes in pf/item/achamo, since Achamo is Torchic's Japanese name. In the Shiny PokéMon Pack, you have to rename every file that says, for example, "Shinytorchic," so that it matches its actual name on the ISO, in this case, "PkmAchamoBrres.pac.

Assist Trophy Characters also follow a similar rule, but their file must be called Asf*ASSIST NAME*Brres.pac
To make your life easier (and mine a bit more difficult for the next 3 and a half hours that I'm typing this), here are the proper names for each PokéMon AND Assist Trophy.

Do item hacks just go in the same folder, or do they replace the existing common3.pac file?

Item files are stored in the Common3. you need BrawlBox to open the archive.

So I edit the files in BrawlBox, save the common3 file from there and then place it back into my folder?  Does this work with Gecko OS?

Yes, open common3.pac, find and replace a file,save and load brawl with via gecko os or riivolution.
