i need help with patching???!!

ok ive gotten 3ds max 8...i know how to make and edit the polygon and all that fun stuff but..i tried patching it and when i patched the polygon it only gets bigger and more didtorted no matter how many times ive patched the polygon....ive watched mariokart64n's tutorial also and that didnt work..so if any one can post the steps thourghly on how to patch or can tell me what i am doing wrong please let me know...

after i patch it..it only gets bigger and more distorted....please help!!
